Who can apply for coverage under this Group Benefit Plan?
The program is available to all everyone in the agricultural, rural or food sectors, including farm families, employees, partners and associate memberships.

How do I enroll under the Group Benefit Plan?
Simply select a plan of interest under the Programs Choices and download all necessary forms required to apply for coverage. You may also obtain information by contacting HUB Administration at 1.800.463.2555.

Coverage is based on a short medical questionnaire contained on the Application Form.

All premium withdrawals will be facilitated through ACC FARMS.

When will my coverage take effect?
Coverage will take effect on the first of the following month after the approval by Co-Operators Life Insurance Company.

When does my coverage terminate?
All coverage with the exception of Long Term Disability and Optional Life terminate on the 1st of July following your 70th birthday. Long Term Disability and Optional Life terminate at age 65.
